Ohio nurse practitioner collaboration is documented through a written standard care arrangement, often shortened to SCA. The Ohio Association of Advanced Practice Nurses (OAAPN) maintains a Standard Care Arrangement resource page and offers a paid model SCA template that NPs can use as a drafting reference.
For practical examples, several Ohio organizations have made standard care arrangements publicly available. These examples can help show how different clinical settings structure collaborator information, scope of services, consultation and referral processes, prescribing provisions, and quality assurance.
OAAPN's page explains the model SCA template, purchase options, and common SCA questions for Ohio APRNs.

Nationwide Children's Hospital provides a public APRN standard care arrangement example from Columbus, Ohio. The example is available here.

This public family-practice example shows how an Ohio SCA can be organized for an outpatient clinical setting. The example is available here.

This public example provides another Ohio SCA format that can be compared with the OAAPN model and other public examples. The example is available here.
When using any template or example, check it against current Ohio law and rules. Ohio requires a standard care arrangement to be developed by the APRN and each collaborating physician or podiatrist and to meet the requirements of Ohio Revised Code section 4723.431 and Ohio Administrative Code Chapter 4723-8.1
The arrangement should identify the parties, show signatures and execution/review dates, list each collaborator's specialty or practice area and contact information, describe the APRN's services and prescriptive scope, and explain how consultation, referral, coverage, disagreement resolution, quality assurance, chart review, prescribing safeguards, Schedule II use, opioid prescribing for minors, and OARRS review will be handled.2
For quality assurance, Ohio requires each participating APRN to take part in a process that includes periodic random chart review at least annually, including prescribing patterns and Schedule II prescribing when indicated, plus an opportunity for the APRN and collaborator to discuss feedback and evaluate patient care.3
