What are the requirements for terminating a collaborative agreement in Connecticut?

In Connecticut, terminating a collaborative agreement means the APRN has completed at least three years (2,000 hours) of collaboration and is electing to practice independently; the APRN must submit written notice to the Commissioner of Public Health of the intention to practice independently.1

After termination, APRNs must maintain documentation of completing the collaborative practice requirement for at least three years and provide it to the Department of Public Health within 45 days upon request.1 APRNs must also provide a copy of the collaborative agreement to the Department upon request, though the Department does not collect agreements on a routine basis.2

Note: APRNs are required to practice under a written collaborative agreement for the first three years after initial licensure before they may transition to independent practice.3
