The requirements for terminating a collaborative agreement in Kentucky include specific steps depending on the agreement type. For controlled substances under a CAPA-CS, either party may rescind the agreement with 30 days’ written notice via registered mail, and the APRN must notify the Board with a copy of the rescission; the Board will then notify the Kentucky Board of Medical Licensure2. If a CAPA-CS ends unexpectedly, the APRN must cease prescribing controlled substances until the agreement is resumed or a new CAPA-CS is executed1.

For nonscheduled legend drugs under a CAPA-NS, if the agreement ends unexpectedly for reasons outside the APRN’s control (e.g., physician termination without notice, loss of license, or death), the APRN may continue prescribing for up to 30 days after documenting in each patient’s record why continued prescribing is justified; there is no extension of this grace period1. APRNs must report any changes to a CAPA-NS or CAPA-CS to the Board within 30 days, and submit required documents through the Board’s website upload system345.
