What is a collaborative practice agreement called in Connecticut?

In Connecticut, a collaborative practice agreement is called a written collaborative agreement between the APRN and a Connecticut-licensed physician for the APRN’s first three years of licensure1. The supervisory model is termed “collaboration”, defined in statute as a mutually agreed-upon relationship aligned with the APRN’s work1.

By law, the written collaborative agreement must address the following1:

  • A reasonable and appropriate level of consultation and referral
  • Coverage for patients in the APRN’s absence
  • Methods to review patient outcomes and medical therapeutics, including tests and procedures the APRN may order
  • Disclosure of the APRN–physician relationship to the patient
  • The level of Schedule II and III controlled substances the APRN may prescribe

APRNs must provide a copy of the collaborative agreement to the Department of Public Health upon request; the department does not collect agreements on a routine basis2.
