In Kentucky, a collaborative practice agreement is called a Collaborative Agreement for the APRN’s Prescriptive Authority—either a CAPA-NS for nonscheduled legend drugs or a CAPA-CS for controlled substances12.

The CAPA-NS is a written agreement with a Kentucky-licensed physician that is required during the first four years of nonscheduled prescribing to the APRN’s population focus, and it must include specified elements on the Common CAPA-NS form13.

The CAPA-CS uses a standardized Form CAPA-CS provided by the Board and must set out collaboration and communication terms, any limits on controlled substances, and signatures of both parties24.
