
In Michigan, a collaborative practice agreement is called a “written authorization.” This document is used when a physician delegates controlled-substance prescriptive authority to an APRN12. By contrast, APRNs may prescribe nonscheduled (noncontrolled) prescription drugs without physician delegation3.
A written authorization must include: (1) the name, license number, and signature of the supervising physician; (2) the name, license number, and signature of the APRN; (3) any limitations or exceptions to the delegation; and (4) the effective date of the delegation1.
The delegating physician must review and update the written authorization at least annually and note each review date on the document4. The physician must also keep the written authorization at the physician’s primary place of practice and provide a copy to the APRN5.
