A collaborative agreement in Michigan must be reviewed annually. Specifically, the delegating physician must review and update the written authorization each year from the original effective date or the date of its last amendment, and must note each review date on the authorization1.

This written authorization is the document used to delegate controlled-substance prescribing and must include the physician and APRN names, license numbers, signatures, any limitations, and the effective date2.
