To end a collaboration, use the Tennessee Department of Health’s process for supervising physicians to terminate the existing relationship, which includes indicating a reason for termination1. Additionally, either the physician or the NP must remove the supervisory relationship in the Controlled Substance Monitoring Database (CSMD)2.

NPs must also update their Board notice when collaborator information changes3, and Tennessee rules require that such updates be made within 30 days of any change45. These steps help ensure prescriptive authority records, protocols, and controlled substance monitoring information remain accurate and compliant.
